These Terms & Conditions ("Terms") govern your use of mysurewell.com (the "Site") and the services of Surewell, Inc. ("Surewell," "we," "us," or "our"), a licensed insurance agency in Illinois. By using the Site or communicating with us, you agree to these Terms.

1. About Surewell

Surewell is an independent insurance agency that helps individuals understand and enroll in Medicare, Medicaid, ACA marketplace, and life insurance coverage. We are not affiliated with, nor endorsed by, any government agency or the federal Health Insurance Marketplace.

2. Informational content only

Content on the Site is provided for general information and does not constitute insurance, legal, tax, or medical advice. Plan availability, benefits, and pricing are set by insurance carriers and government programs and may change. Nothing on the Site is an offer or guarantee of coverage.

3. No guarantee of outcomes

We work to match you with suitable coverage, but eligibility and enrollment decisions rest with the insurance carriers and programs, not with Surewell. We do not guarantee any particular plan, premium, or approval.

4. Your responsibilities

You agree to give accurate information and to use the Site and our services lawfully. You are responsible for reviewing plan documents provided by carriers before you enroll.

5. Intellectual property

The Site, its text, and its design are owned by Surewell and may not be copied or reused without our permission.

6. Privacy

Our handling of your information is described in our Privacy Policy, which forms part of these Terms.

7. Text messages (SMS)

Surewell may send text messages to people who have given us their mobile number in the course of working or doing business with us — for example, staff and representatives who receive operational messages such as document requests, reminders, and account notices. We send these only to people who have provided their number and agreed to be contacted this way.

• Consent. By giving us your mobile number for this purpose, you consent to receive text messages from Surewell related to our operations and your relationship with us. Consent to receive texts is not a condition of any purchase.
• Message frequency. Message frequency varies. Message and data rates may apply.
• Opt out. You can opt out at any time by replying STOP to any message; we will stop texting that number. Reply HELP for help, or contact us using the details below.
• Carriers. Mobile carriers are not liable for delayed or undelivered messages.
• No sharing. No mobile information, and no text-messaging opt-in data or consent, will be shared with any third parties or affiliates for any purpose, including marketing or promotional purposes. See our Privacy Policy.

8. Changes to these Terms

We may update these Terms from time to time. The effective date above shows when they last changed. Continued use of the Site after a change means you accept the updated Terms.
